Job Description:
The Community Integration Home (CIH) is an outpatient service of Georgia Regional Hospital that provides care and treatment to patients with a mental health diagnosis and forensic status. Perform practical nursing care and related technical work in the care, treatment and rehabilitation of individuals with a mental health diagnosis. Full Performance employees are provided general instructions, and specific instructions or immediate supervision in cases requiring advanced and/or specialized treatment and procedures; perform work independently, receiving specific guidance only in new, unusual or emergency situations; and receive periodic review of work to ensure that acceptable practices are followed; and perform other related duties as assigned.
The selected candidate must also be able to perform the following:
Job Responsibilities:
Manages the medication area, medication orders and coordinate with community-based pharmacist regarding inventory, prescriptions,
Develops and maintains accurate documentation of patient medical administration and health records.
Monitor and report changes in patient symptoms or behavior.
Communicate with collaborating physicians and CIH leadership team regarding resident care.
Administer nursing care to behavioral health patients.
Coordinates medical services as needed and required.
Educate residents about health maintenance and behavioral health diseases.
Facilitate referrals to other healthcare professionals and medical facilities.
Ensures that each resident receives the required care regarding physical health and dental services within the appropriate time frames.
Provides education to other direct care staff on a quarterly basis or as needed based on needs of the residents.
Provide advice and emotional support to patients and their family members.
This position requires traveling throughout DBHDD’s assigned work locations of Stone Mountain, GA and Douglasville, GA. The selected candidate will be provided job related equipment such as a work cell phone and a laptop computer. In addition, the selected candidate will receive mileage reimbursement for work related travel.

Ability to clearly explain test procedures to consumers.
Ability to participate in supportive guidance, program planning meetings, consumer staffing and service trainings.

Ability to utilize basic IT software programs, including Microsoft Suites.
Ability to operate IOS phone and Dell computer programs.
Qualifications
Vocational/Technical degree from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse AND Six months of nursing experience.
Preferred Qualifications
Two Years’ experience working with individuals with a mental health diagnosis in an inpatient or outpatient setting. Two years previous work experience working with individual with mental illness, substance use, and developmental disabilities substance abuse individuals.
