Provide direct nursing care appropriate for a licensed RN.
Participate in the development of the patient’s plan of care and coordinate the services and activities of the multidisciplinary team in respect to the plan of care.
Document activities and assessments in the clinical information system and appropriately updates team members.
Provide support to patient’s family/friends including education, expectations, and reporting changes in condition.
Support the organization’s goals, initiatives, and value system.
Understand and is compliant with organizational policies and processes relating to regulatory, accreditation and privacy standards.
Graduate of an accredited school of nursing, Bachelor’s degree preferred.
Current Texas RN license.
Current certification in basic life support.
Ability to function as part of an interdisciplinary team.
Good interpersonal skills – able to effectively relate and communicate with team members and patients from a diverse community.
Knowledge and experience in a hospice or hospital setting is preferred.
Experience using a clinical information system ERP is preferred.
